Kathy DiFrancesco, BSN, RN, CHPN, MS, is a highly experienced registered nurse and Geriatric Care Manager with more than 12 years of expertise in hospice, palliative care, and healthcare administration. She is a Certified Hospice and Palliative Nurse (CHPN) and holds a Master of Science degree in Palliative Care Administration from the University of Maryland.
Throughout her career, Kathy has devoted herself to serving individuals and families during some of life’s most profound and vulnerable moments. Her extensive clinical and leadership experience has provided her with a deep understanding of the emotional, spiritual, and practical challenges that often accompany aging, serious illness, caregiving responsibilities, and end-of-life transitions.
Kathy is committed to creating a compassionate and supportive environment where patients, families, and caregivers feel heard, respected, and empowered. She believes every individual deserves dignity, peace, comfort, and clarity throughout their end-of-life journey. Through thoughtful guidance and compassionate communication, she helps families navigate difficult conversations and complex decisions with confidence and understanding.
